GSMC EVENT SPECIAL NEEDS REQUEST FORM

This form will assist us in helping you and your Scouts have the best possible time at this event, so please be as detailed as possible. After this form is received, it will be reviewed by our Council personnel, and we will contact you if we need additional information.

Submitting a copy of the participant's Annual Health and Medical Record (AHMR) Parts A, B, and C attached will greatly assit us in our efforts to provide a positive experience.  You may securely upload a copy using this link.
