
This form is to be used to request a Certificate of Insurance for your outing, event, or meeting place. If you are visiting another Scout camp and are requesting a certificate of insurance, please use the Description of Coverage sheet under Insurance Forms located at https://lhcscouting.org/resources/
THIS INSURANCE POLICY IS EFFECTIVE 3/1/2025 - 2/28/2026
Please allow TWO WEEKS for the processing of your request.
The certificate holders are listed as additional insured as part of the language already available on the document.
Modifications to the document must be cleared by the national insurance carrier and are at the discretion of the National Organization. If a certificate needs to be sent to the national organization, the process can take up to a month or longer to recieve back.
